The 22-year-old Italian had to come from behind to win 6-7 (8-10) 6-3 6-2 in two hours 22 minutes.
The Australian Open champion sealed victory on his first match point and next faces Hungary's Fabian Marozsan.
"It was mentally tough," Sinner said.
"I was 5-1 up in the tiebreak in the first set. But this can happen, especially on this surface, it can go very, very fast. I think I was good mentally."
Germany's Alexander Zverev, runner-up at the French Open, also had to come from behind to progress to the second round as he got used to grass again.
He defeated compatriot Oscar Otte 6-7 (5-7) 6-3 6-4 and next faces Lorenzo Sonego of Italy.
Zverev, aiming to go one step further than his two finalist appearances in Halle (2016, 2017), withdrew from last week's Stuttgart ATP grass event after his long run at Roland Garros.
Fourth seed Andrey Rublev continued his patchy form with a straight-sets loss to American Marcos Giron, but sixth seed Stefanos Tsitsipas began his grass season in winning fashion, ousting home hope Henri Squire 7-6 (9-7) 7-6 (7-2).
The Greek saved a set point in the first-set tiebreaker, dropped just three points behind his second serve and did not face a break point throughout the hard-fought 92-minute match.
The 25-year-old is aiming for his second grass trophy (2022 Mallorca) and 12th career ATP Tour trophy. This week marks Tsitsipas' fourth appearance in Halle but he is yet to move past the second round at the ATP 500 event.
Qualifier James Duckworth scored a 6-4 6-4 victory over Brazilian wildcard Joao Fonseca but fellow Australian Max Purcell was beaten 7-6 (7-1) 6-4 by seventh-seeded Kazakh Alexander Bublik.
